ITDN Form 2

 CATEGORY 2 MEMBERSHIP

Form 2 below must be used ONLY by tap dance teachers or assistant-teachers who have taught for three years or more. If you are a teacher who still considers herself/himself a "student of tap" (most good teachers do!) or a professional or retired professional performer who now makes a living mostly by teaching, you should still fill out Form 2, that's OK.
 

Reminders
 You should NOT fill out Form 2 on this page if you are a tap dance student, fan, tapper's parent, assistant-teacher (for less than 3 years,) former tapper, etc. Instead you should fill out Form 1.

You should NOT fill out Form 2 on this page if you are a studio owner, tap program director, etc. Instead you should fill out Form 3.

Membership status  [required]

M1 - I am a brand new member, I had never filled out this form before.
M2 - I am already on your membership list (I had used this page/form to join) but some of my contact info (email address, phone number or mailing address, etc.) has changed - or - I would like to update/upgrade my membership to join the ITDN "SmartSavers Tap Dance Club." Please update your database.
M3 - I had previously filled out another form by mistake. Please delete that info from your database and enter me on your list as a new Category 2 member.
Make SURE that you submit a new membership form and check the M2 button when your email address or street address changes. Bouncing email addresses and returned mail result in all info being deleted from the database and membership being revoked. You would then also become unable to receive ITDN discounts or notification of ANY upcoming event!

 General membership information is available here. Please follow these three easy steps.

1) Membership type (check one)
 Annual membership fee, solo: $15 per year
Annual membership fee, family*, two people: $22
Annual membership fee, family*, three people: $29
Annual membership fee, family*, four or more people: $35

* By "family" we mean 1st degree, immediate relatives, such as parents, children, brothers or sisters. More distant relatives such as grandparents, grandchildren, cousins, etc. must use the "solo" membership rate.

2) Type here the names and relationships of all SmartSavers Tap Club members to be enrolled
Do not forget to enter their names accurately so that SmartSavers Tap Dance Club discounts may be applied to workshops/conferences/seminars they may wish to attend! Please start with the adult(s) when applicable (i.e. "Melanie Smith, mother")



 I understand that my membership will not be accepted and processed and my payment will not go through unless/until I reply to an official email asking me to confirm that my email address is valid.

3) Choose your type of payment

4) Choose your membership renewal (check one, don't forget!)

 Easy, automatic renewal
Please renew my SmartSavers Tap Club membership automatically each year on the date my membership was processed (reminder: this can only be done after you have confirmed your email address.) I understand that should the membership rates ever go up, I will be asked again via email if I wish to keep this option.
 "Manual" renewal
Please email me to ask if I wish to renew my SmartSavers Tap Club membership in the four to eight weeks preceding its expiration.
